    Milia en plaque (MEP) was a rare dermatological condition characterized by numerous tiny milia on an erythematous base, typically occurring in adults around the peri-auricular areas. This case report described a unique presentation of MEP on the scalp of a 5-year-old boy, present since birth, leading to circumscribed cicatricial alopecia. The condition was resistant to treatments such as minocycline and isotretinoin. The congenital presentation and Blaschkoid extension suggested an in-utero tissue alteration, offering new insights into the pathogenesis of MEP. Despite various medical and surgical treatment attempts, successful management of MEP remained challenging.
