TLDR 2.5 mg/mL TAC is as effective and safe as 5 mg/mL, but 5 mg/mL may improve hair regrowth and density more.
This study compared the efficacy and safety of two concentrations (2.5 mg/mL and 5 mg/mL) of intralesional triamcinolone acetonide (TAC) in treating alopecia areata (AA) in 60 patients, divided equally into two groups. Both groups received monthly TAC injections for three sessions. Results showed no significant difference in hair loss area and SALT scores between the groups after 12 weeks. However, the 5 mg/mL group showed better outcomes in terminal hair regrowth (40.7% vs. 27.9%) and hair density improvement (59.3% vs. 42.7%) compared to the 2.5 mg/mL group, with statistical significance (p < 0.05). Adverse effects were more common in the 5 mg/mL group (43.4%) than in the 2.5 mg/mL group (20%). The study concluded that 2.5 mg/mL TAC is as effective and safe as 5 mg/mL, but the higher concentration may be more effective for hair regrowth and density.
