This case report discusses a 48-year-old man with systemic lupus erythematosus (SLE) who presented with persistent proteinuria and hematuria. A renal biopsy revealed isolated C3 glomerulopathy with active lesions, and further investigation showed a low C3 complement level and a monoclonal IgG Lambda component. Despite normal glomerular filtration rate, these findings suggest a diagnosis of monoclonal gammopathy of renal significance rather than typical lupus nephropathy. The report emphasizes the importance of considering atypical presentations in SLE patients to expand diagnostic possibilities.
