TLDR BPH medications may increase the risk of self-harm and depression, especially in the first 18 months.
A study published in JAMA Internal Medicine on March 20, 2017, examined the mental health risks associated with 5α-reductase inhibitors (5ARIs) like finasteride and dutasteride in older men. The study involved 93,197 men aged 66 and older in Ontario and found no increased risk of suicide but did identify a higher risk of depression and self-harm, particularly within the first 18 months of use. Specifically, there was a 1.94-fold increased risk of self-harm and a 1.88-fold increased risk of depression during this period. After 18 months, the risk of self-harm normalized, but the risk of depression remained slightly elevated. The study highlighted the importance of recognizing these potential side effects while also noting that the benefits for prostate health should not be overlooked.
