TLDR A 4-year-old boy's itchy, scaly scalp and hair loss were correctly diagnosed as tinea capitis after initial misdiagnosis.
A 4-year-old African American boy presented with an itchy, scaly scalp and patches of hair loss, initially misdiagnosed and treated with limited success using topical antifungal cream and ketoconazole 2% shampoo. Physical examination revealed multiple patches of hair loss and a scaly scalp with black dots. Microscopic examination confirmed the presence of large-spore endothrix fungus within hair shafts, diagnosing tinea capitis.
