Evidence-Based Data on Duration of Baricitinib Treatment for Severe Alopecia Areata

TLDR Baricitinib treatment for severe alopecia areata shows significant improvement by 52 weeks, especially in very severe cases.
The study evaluates the optimal duration of baricitinib treatment for severe alopecia areata (AA) using data from the BRAVE AA1/AA2 phase 3 trials. Patients were treated with 4 mg of baricitinib daily for up to 52 weeks, and their responses were assessed based on baseline severity and episode duration. For those with severe AA (SALT score 50-94), improvement (SALT30) was observed between 24-36 weeks, with 75% achieving a SALT score ≤20 by week 52 if their episode was <4 years, compared to 65% for episodes >4 years. In very severe cases (SALT score 95-100), improvement continued through 52 weeks, suggesting that treatment duration should be tailored to baseline severity, with at least 1 year needed for very severe cases to see significant effects.
