69-Year-Old Man With Dysuria and Right Lower Abdominal Pain

    TLDR The man was diagnosed with stage III multiple myeloma and treated to improve kidney function.
    A 69-year-old man with a history of benign prostatic hyperplasia, obesity, hypertension, and hyperlipidemia presented with dysuria and right lower abdominal pain. Initial tests indicated acute kidney injury and pyelonephritis, but further investigation revealed stage III IgG kappa light chain multiple myeloma (MM) with light chain cast nephropathy (LCCN). He was treated with cyclophosphamide, bortezomib, and dexamethasone, with plans to add daratumumab post-plasmapheresis to reduce light chain production and improve renal function. The patient was considered for autologous hematopoietic cell transplantation (HCT) and received prophylactic antimicrobial and antithrombotic therapies.
