Five-Year Results of a Prospective Very Low-Calorie Diet or Conventional Weight Loss Program in Type 2 Diabetes

    TLDR The intensive conventional diet with exercise led to better long-term weight loss and health improvements than the very low calorie diet in obese type 2 diabetics.
    The study compared the long-term effects of a very low calorie diet (VLCD) and an intensive conventional diet (ICD) with exercise on obese type 2 diabetic subjects over 5 years. Forty-five subjects with a BMI over 30 were divided into three groups: 15 chose VLCD, 15 chose ICD, and 15 did not follow either program. Initially, the VLCD group lost weight faster, but the ICD group maintained better weight loss after 5 years, with a mean weight loss of 8.9 kg, improved HDL cholesterol levels, and reduced diastolic blood pressure. The VLCD was effective for quick weight loss, which could be beneficial before major surgery, but weight regain was common. The ICD group showed that lifestyle changes can lead to sustained weight loss, but none achieved complete remission of diabetes. The study highlights the importance of long-term strategies for weight management and increased physical activity in managing type 2 diabetes.
