TLDR Antituberculous drugs improved symptoms in a woman with lupus and myositis.
A 39-year-old woman with systemic lupus erythematosus (SLE) experienced persistent symptoms despite treatment, leading to a diagnosis of tuberculous myositis, which mimicked autoimmune myositis. Her condition improved significantly with antituberculous drugs, allowing for reduced immunosuppressive therapy. This case highlighted the difficulty in distinguishing between infection and autoimmune disease in immunocompromised patients and emphasized the importance of considering infection in atypical presentations and the utility of muscle biopsy for diagnosis.
