TLDR The boy's symptoms improved with vitamin C treatment, highlighting the need to consider scurvy in kids with unusual diets.
A 14-year-old boy presented with lower extremity swelling, petechial rash, and other symptoms, leading to a diagnosis of vitamin C deficiency (scurvy) after extensive evaluation. Despite initial consideration of conditions like Henoch-Schönlein purpura, syphilis, osteomyelitis, and Lyme disease, the chronicity of symptoms, specific dermatologic findings, and low vitamin C levels confirmed scurvy. The boy's restrictive diet and oral aversion contributed to multiple vitamin deficiencies, including vitamin C, zinc, and others. Treatment with ascorbic acid and other supplements led to significant improvement in symptoms within 10 days. This case highlights the importance of considering scurvy in pediatric patients with atypical diets and musculoskeletal symptoms.
