Baker′s dozen on the scalp: An interesting case of multiple trichilemmal cyst

    TLDR A woman had 13 non-cancerous cysts on her scalp successfully removed in one surgery.
    In 2014, a case study was reported involving a 55-year-old female with 13 trichilemmal cysts (TCs) on her scalp, which are benign cystic swellings originating from the hair follicle epithelium. The patient had no family history of similar conditions, and the cysts varied in size from 1 × 1 cm to 3.5 × 3 cm. All cysts were excised in one session under local anesthesia, and histopathology confirmed the diagnosis of TCs, which are characterized by a specific form of keratinization and lack of a granular layer in the cyst wall. TCs are common, affecting 5-10% of the population, often multiple, and usually found on the scalp. They can be confused with epidermoid cysts but lack a punctum and have distinct histopathological features. The case highlighted the ease of surgical excision of TCs due to a clear plane of cleavage between the cyst wall and surrounding tissues.
